Clark County Combined Health District records are available for the public because of Ohio Sunshine Laws and the Freedom of Information Act. These policies cover all the divisions of CCCHD.

To request a public record, contact our Public Records Custodian, Shannon Hackathorne at 937-390-5600 ext. 291 or emailing shackathorne@ccchd.com.

Responses to requests will take place during regular business hours of 8 a.m. and 5 p.m.  Monday through Friday. Verbal requests are acceptable; however, requests in writing may help the Public Records Custodian in getting the requested documents more easily.
